Patient Resources

What to bring to your Appointment

  • Your driver’s license or other photo identification
  • Your insurance cards
  • Physician referral forms if required by your insurance
  • A list of current prescriptions as well as over-the-counter medications you are taking, including doses and frequency
  • Information about your medical and surgery history
  • Any recent labs, x-rays, MRI’s CT’s or other records you may have. You may also be required to bring the actual disc from your studies as well as the written reports
  • Any office visit copay amounts

Office Policies

Texas Health Surgical Care accepts most major insurance plans. We will bill your insurance, however any amounts not paid by insurance is the responsibility of the patient.

After your insurance pays, we will begin billing you for unpaid balances such as deductibles and co-pay amounts. Should you have questions about what your insurance will cover, please contact your insurance company. Please contact us if you wish to discuss payment plans and options.

Depending upon your insurance coverage, we might require prepayment of expected patient balances.
